2010 ACCEA applications
We would like to inform you that the online system for the 2010 round of ACCEA applications
will be available on the ACCEA Website at the end of September 2009 at
http://www.dh.gov.uk/ab/ACCEA/index.htm
The closing date for receipt of all applications, ranked lists and citations will
be Friday 11th December 2009.
If you would like BMFMS to submit a citation on your behalf please send us a copy
of your submitted application form by the 5th October 2009 to bmfms@rcog.org.uk

Attached, please find set of application forms for you to complete and return to
help us assess your application including BMFMS member scoring form.
Bronze or Silver candidates may choose to submit ONE of the supplementary forms
below with their application form. Gold candidates may choose to submit up to TWO
of the forms below.
Should a candidate choose to submit a supplementary form, the corresponding domain
in the application form must be left blank.
